Healthy Mountain Living

Do you want to be healthier, but don't know where to start? Do you need more guidance developing a plan for a healthier you?  The Healthy Mountain Living Self-Care Case Management program may be what you're looking for!  Call us to schedule a Health Risk Assessment, which will rate your risk for cardiovascular disease.  If you have an elevated risk, you can receive cholesterol and glucose testing free of charge.  A Registered Nurse Case Manager will work with you to set goals for reducing your risk and improving your heart health.  Exercise and nutrition consultation, tobacco cessation resources, medical referrals and one-on-one counseling will be available to you.
