The Divide Health Clinic provides exceptional healthcare for people facing access barriers.

How to Register

Patients must register with Peak Vista to receive medical and /or dental services.  If you are not already registered, please call 719-228-6667, Monday through Friday between 8:00 am and 4:30 pm, except for Mondays which fall on federal holidays.  Registration is on a first-come-first-serve basis.  

Appointment Scheduling

Teller and Park County patients are able to schedule appointments directly with the Divide Health Clinic Monday - Friday 8:00 am to 5:00 pm by calling 719-687-4460.

Emergencies

If you have a life threatening emergency, call 911.  In case of non-life threatening emergencies, contact a Peak Vista registered nurse at 719-632-5700 before going to a hospital emergency room to avoid unnecessarily paying a hospital bill.  A nurse is available at this number 24 hours a day, 7 days a week.

Pharmacy Services

For our patients without insurance, the Divide Health Clinic provides sliding fee pharmacy services through our main Peak Vista pharmacy.  Prescriptions from the main pharmacy arrive via courier and are available for pick-up at the Divide Health Clinic from 8:00 am to 4:30 pm, Monday through Friday (except holidays).
