File of Life

You can get your "File of Life" at the Teller County Sheriff's Office

The File of Life is designed to attach to your refrigerator with a magnet so that in times of emergency, responders can find important information:

  • A place to indicate if you have a Colorado Advanced Directive or a DNR form and instructions to attach the form.
  • Emergency Contacts
  • Special Medical Conditions - Latest Data
  • Medications (including dosage and frequency)
  • Recent Surgeries
  • Other Existing Medical Conditions
  • Allergies
  • Medical Insurance

The File of Life card is made to fold and be placed into a red plastic sleeve with a magnet on the back.  Emergency personnel are trained to look for these red sleeves when they respond to an emergecy.
