Jacqueline Davis RN, MA

Jacqueline is the case manager for our Healthy Mountain Living Self Care Case Management program.  As such, she works with clients to identify their risk factors for chronic disease, develops an action plan to reduce and/ or eliminate risk and then counsels their progress. She is also a Be Well Colorado facilitator that meets out in our communities with small to moderate size groups of people to address chronic disease self care.
